Welcome to "My Guide for Living with Dementia". If you would like to know about the development of "My Guide" go to the About the project page under Me & U-First! in the sidebar. Overview of My Guide for Living with Dementia
"My Guide for Living with Dementia" is a communication, information and care planning tool about and for the Person with dementia and their Partners in Care. "My Guide" will: Help with communication between you, your family, and others involved in supporting you. Inform you about dementia and the many factors that can influence your dementia Help you to record valuable information Give you tips for planning for the future
The guide includes: INTRODUCTORY BOOKLET: defines key terms & use of the guide, introduces the P.I.E.C.E.S. model "GETTING TO KNOW ME": place to fill in important information P.I.E.C.E.S.: 6 subsections provide information, tips and opportunities to record important details about the Person with dementia within the P.I.E.C.E.S. framework PARTNERS IN CARE: information to help Partners to support the Person with dementia. RESOURCES: Handy Contact List

Printing instructions for all or part of the Guide
You can download and print the entire "My Guide for Living with Dementia" or sections as you need them. To print individual pages, when viewing the Guide on the screen make sure your cursor is on the page that you want to print. When you are in the printing dialogue box, select 'Current page'. If printing more than one page but not the whole document - select the page range.
The full Guide, including the introductory booklet, is 156 pages and is available in one large file below all the individual sections.

Hard copies of this booklet are available free from the Alzheimer Society offices in Barrie and Orillia and the North Simcoe Muskoka CCAC. It is also included in the hard copy binder. You can download a copy here. It includes the introductory "Getting to Know Me" page.

You have two choices here. 1) You can print only the Table of Contents which tells you where to find each Getting to Know me page found within each section of the Guide. As you read through the guide, fill in the "Getting to Know Me" pages and move them into this "Getting to Know Me" section. Or, if you prefer to print new pages at a later date, you can return to this web site to print the "Getting to Know Me" pages that as you need them 2) You can print all the "Getting to Know Me" pages including the Table of Contents. For your convenience, all the "Getting to Know Me" pages found throughout the Guide are consolidated in this section. Print the whole section or individual pages as you need them.

Your Social history will influence your experience with dementia. It is important for your Partners in Care to understand your life experiences. This could include your values, cultural background, significant life events, and important relationships. This section includes many Getting to Know Me pages to help you share this important information. Total of 20 pages.

This part of "My Guide for Living with Dementia" is written for the Partners in Care. Partners in Care refers to the family member, the friend, the professional - who are involved in supporting the Person with dementia. Download this section if you are part of a caregiving team. It is 17 pages.

This section is only four pages, and contains a blank form so that you can create a Handy Contact List. This is a place for you to keep important documents, such as notes from specialists, reports, information about services and tips from your Partners in Care. This list can also be found in the Getting to Know Me section.
