The North Simcoe Muskoka Dementia Network

The creation of Local Health Integration Networks and the realignment and integration of services for dementia care have created a natural alignment between services and agencies in Muskoka and those in North Simcoe County.
To achieve "one voice" within the North Simcoe Muskoka Local Health Integration Network (LHIN 12)the Dementia Networks of Simcoe County and Muskoka have completed a joint strategic planning initiative to identify future directions and establish a framework for collaboration.

A Brief History

A strategic planning session was held 1 year ago involving the Dementia Networks of Muskoka-Parry Sound and Simcoe County. We engaged a facilitator, David Sheridan, to lead us through the process. A discussion paper was produced and shared widely for comment. An action planning session was held in October and a plan finalized late in 2007. One key recommendation was to amalgamate the two Networks in the North Simcoe Muskoka LHIN. An interim steering committee, established after a call for interest, drafted terms of reference for the Network and the Steering Committee as well as an application process for people to apply to be a member, and planned a Forum for May 30, 2008. The call for Steering Committee membership was circulated at and following the Forum and the permanent members of the North Simcoe Muskoka Dementia Network Steering Committee (3 Alz Soc chapters + 3 PRCs) met by teleconference to review the applications and consider the applicants’ perspectives for the inaugral Steering Committee.

We are excited about this new network and the opportunity to work together.
Our plans will be guided by our strategic plan which is attached below and by many of the LHIN iniatives.
