Your PRCs in Simcoe County

The Psychogeriatric Resource Consultants have been active in their positions in Simcoe County since August 13th, 2001. An advisory committee with diverse geographic and sector representation ensures that the PRCs in Simcoe County continue to follow their original three part mandate:
Community Development, Education and Support/Consultation

Background

The Ontario Strategy for Alzheimer Disease and Related Dementias was “the first comprehensive multi-faceted provincial strategy on Alzheimer Disease to be introduced in Canada”

As part of this 10 point strategy, more than 50 Psychogeriatric Resource Consultants provide service across Ontario in Long-term Care homes, Community Agencies and CCACs. This is a “crucial step toward improving the quality of life for Ontarions affected by Alzheimer Disease and related dementias. The strategy is paramount to ensuring that seniors can live with dignity and comfort”

The PRCs, Initiative #8 of the 10 initiatives, are integrated with the other Alzheimer initiatives in Simcoe County including:

  • Initiative #1 – The PIECES trained persons in long-term care homes and the community who are working
    directly with persons with cognitive/mental health needs.
  • Initiative #3 - Alzheimer Society Public Education Co-ordinators, who are providing public awareness, information and education to the general public, professionals and families affected by Alzheimer Disease and related dementias.
  • Initiative #9 – Dementia Networks with the goal to improve the system of care required by persons with dementia, their families and caregivers. This includes all service providers and organizations that enhance the well-being of persons with dementia, their families and caregivers.

  • The Role of the PRC

     Our Role - Community Development, Education and Support/Consultation
     to provide on-going support to LTC Home staff, CCAC Case
    Managers and community agencies who have been trained
    with the PIECES and/or U-First!training package.
     to provide education and training in a variety of formats to meet identified needs.
     To assist in the development and maintenance of local agency networks that will identify gaps and integrate services to meet client/staff needs in crisis situations.
     To also provide assistance in education/training, care and treatment planning

    Enhancing consistency through the PIECES program

  • PIECES program has been initiated by ‘PIECES’ trained persons in LTC and CCAC throughout the province
  • Psychogeriatric Resource Consultants work with front line staff to reinforce the PIECES program, whether or not PIECES trained
  • PIECES resource templates will be used throughout the province
  • common language will be used throughout
  • compliance advisors, nurse practitioners and preceptors have also had access to PIECES training
  • PRCs maintain a county wide PIECES network to share information and successes.
