North Simcoe Muskoka Dementia Network Terms of Reference

Mission

The North Simcoe Muskoka Dementia Network exists to improve the quality of life of persons with dementia, their family and caregivers, through advocacy, collaboration, education and innovation.

Our vision is that the person living with dementia will experience:

• High quality, dignified and compassionate services and supports across a seamless continuum of care respectful of diversity
• A system that has adequate and stable resources
• A well integrated health, mental health and social services network that affirms the role of families and communities

To achieve this vision the Network will:


1. Share knowledge amongst persons with dementia, their formal and informal caregivers and any other interested individuals or organizations.

2. Enhance partnerships and linkages across the network of care inside and outside of the North Simcoe Muskoka LHIN.

3. Identify critical needs and issues and review these in light of our Strategic Plan updating as necessary.

4. Make recommendations (local, regional and public policy levels) for addressing the identified needs of persons with dementia, and their formal and informal caregivers within LHIN 12.


Detailed short-term goals and objectives are included in the NSMDN Strategic plan developed in 2007.

Structure

The North Simcoe Muskoka Dementia Network is an open, inclusive group of individuals and agencies that are committed to the stated Mission.

At the core of the Network is a Steering Committee whose members represent the varied geographic and service sector areas of North Simcoe Muskoka. They facilitate the implementation of the Network’s Strategic Plan.

Committees such as Marketing and Education will be established as described in the Network’s Strategic Plan.

  • Chair

    Chair of the Network will be a shared responsibility. Co-Chairs will act as advocates and spokespeople for the Network. Co-Chairs will be chosen by the Steering Committee.

    The term of office will be 2 years and the people chosen will have been part of the Steering Committee for at least a year.

    The Steering Committee will work to ensure that only one Co-Chair leaves office at any one time to ensure continuity in leadership.

    The Chairs will be supported by the Psychogeriatric Resource Consultants who are available to assist with developing, facilitating and maintaining the North Simcoe Muskoka Dementia Network.


    Meetings


    Meetings will be held a minimum of twice a year and as needed to fulfill the objectives of the Network. Meeting locations will be chosen to facilitate the greatest participation from across North Simcoe Muskoka and tools such as teleconferencing and videoconferencing will also be considered to enhance members’ involvement.
