The GiiC Toolkit: A comprehensive on-line toolkit developed by the The Geriatrics, Interprofessional Practice and Interorganizational Collaboration (GiiC) Initiative:Enhancing Shared Care of Frail Seniors
The GiiC initiative is a collaboration of the Regional Geriatric Programs of Ontario located in Hamilton, Kingston, London, Ottawa and Toronto, the Centre for Education and Research on Aging and Health (CERAH) at Lakehead University in Thunder Bay and the North East Specialized Geriatric Services (NESGS) Group in Sudbury. CERAH and the NESGS Interest Group are affiliated with the Northern Ontario School of Medicine. rgps.on.ca/giic

Driving and Dementia Pamphlets:
The Kingston, Frontenac, Lennox & Addington Dementia Network has produced two excellent resources on driving and dementia. One is a pamphlet for persons with dementia and their families summarizing key issues related to driving and dementia, and describing the Ontario Ministry of Transportation (MTO) reporting and review process in a patient-friendly format. The other is a guide for physicians, reviewing the process of reporting potentially unsafe drivers with dementia to the MTO.

We are pleased to announce the launch of "Creating Partnerships in Dementia
Care: A Changing Melody Toolkit".
Over the past 5-years MAREP has systematically evaluated the outcomes and
processes of the A Changing Melody forum, a learning and sharing forum for
persons with early-stage dementia. As a result we have compiled the
information into a practical 177-page resource guide to help plan and
implement forums specifically designed by and for persons with early-stage
dementia and their family partners in care, similar to the A Changing Melody
forum hosted by MAREP and partners.
In addition, we have created a special webpage to support those interested
in hosting their own event. Here you will find resources and a link to an
e-forum where you can share your ideas with others, ask questions and get
answers. This e-forum is hosted by MAREP and the Alzheimer Society of
Canada.
If you are interested in ordering a copy of the toolkit or would like to

Brief Cognitive Screening - Physician Tip Sheet

This tip sheet, including a flow sheet and instructions for the Mini-Cog is a result of consultations with area physicians who expressed the need for a very brief screening tool. For consistency in Simcoe County, The Dementia Network Physician Task Group will promote the use of the Mini-Cog. On April 20, 2005 Dr. Ken Shulman presented a workshop on the Mini-Cog and Clock Drawing Test. This tip sheet is currently being improved. The second page includes a list of dementia supports in Simcoe County.

Vision for an Integrated Seniors' Health Program in North Simcoe Muskoka

This vision was researched and written by Sandra Easson-Bruno under the direction of the Seniors Health Regional Action Group with funding from the North Simcoe Muskoka LHIN through the Aging at Home Program.
It serves as a model for planning and delivering care for seniors in NSM. The Regional Falls Program will begin to unfold summer 2009. The first building block in an Integrated Regional Seniors Health Program for North Simcoe Muskoka is the Regional Falls Program.

This document is a model protocol for community response to adult abuse issues in Simcoe County. This document was revised in 2000, 2003 and 2005.

Specialized Geriatric Services in the Central East Region

This 70 page document was completed in March 2005, in the final days of the District Health Council. Intended as a consultation report, the extensive research was summarized to make it available to stakeholders. Due to time constraints, the document was completed by DHC staff and not vetted by either the DHC or community partners or proofed for accuracy of information. It is provided as a base for further discussion.

Globe & Mail 4/19/15. TORONTO -- "Nursing homes are in dire need of more funding, stiffer regulations and better-trained workers, a coroner's jury has ruled.....
The jury, which had been delving into two homicides at an inquest, is urging Ontario's Ministry of Health and Long-term Care to take a much more hands-on regulatory role. It also urges the deficit-stricken province to "revise the funding system presently in place . . . within the next fiscal year."
A complete overhaul, and an expensive one, is being recommended. These are the recommendations .....
