Terms of Reference, (draft, April, 2008)
Purpose
To guide the work of the North Simcoe Muskoka Dementia Network.
Responsibilities
To plan Dementia Network meetings
To ensure that the activities of the Dementia Network fall within the agreed upon terms of reference for the Network
To pursue the achievement of the goals and tasks laid out in the Dementia Network Strategic Plan.
To initiate, support, monitor and evaluate the activities of the Network as well as any task groups or standing committees.
To communicate effectively with the Network’s membership and ensure adequate and ongoing representation from all geographic and service areas.
To act as a liaison with the North Simcoe Muskoka LHIN
Membership
The Steering Committee will be comprised of but not limited to the following stakeholder groups to a maximum of 18.
3 Alzheimer Society Chapters in North Simcoe Muskoka Psychogeriatric Resource Consultants NSMCCAC representative NSMLHIN representative Consumer/person with dementia LTC Homes Mental Health Services Community Support Services Hospitals Family Health Teams Physicians Retirement Sector Francophone Community Aboriginal Community Members will be chosen based on geographic and sector representation. Individuals may be able to bring multiple perspectives. Initial targets of 1/3 membership from Muskoka will be pursued with a long term goal of balanced representation from the 5 planning areas in the NSMLHIN. Formal and informal partners and caregivers will be asked to advise on activities of the Network through invitation.
Term
Two years with half the committee to rotate each year. (This will be waived in the membership year ending June 2009 in recognition of the Network’s new configuration)
Decision Making
It is intended that a consensus model of decision-making will be used whenever possible and that voting will only be used when consensus cannot be reached. The consensus model will encompass the following parameters: Have all Committee members had the opportunity to discuss the issue? Do members understand the decision to be made? Can members live with the decision? Can members defend the decision? Are members committed to act on the decision?
Leadership
The Co-Chairs of the Steering Committee will lead the Network and act as spokespeople for the North Simcoe Muskoka Dementia Network.
Frequency of Meetings
Meetings will be held every 2 months (or as needed) to facilitate the work of the North Simcoe Muskoka Dementia Network. Meetings may occur face to face, via teleconference or web conference.
Review of Terms of Reference
The Terms of Reference will be reviewed on a yearly basis
